Our evidence-based, community-integrated model emphasizes treating the whole person—addressing physical, emotional, and social aspects—through confidential support, early intervention, nutritional counseling, and partnerships with primary care providers to promote recovery, resilience, and optimal wellbeing for patients, families, and the underserved across Indiana.
